The Center for Advocacy for the Rights and Interests of the Elderly (CARIE) has developed a series of videos to educate individuals about Community HealthChoices. Community HealthChoices (CHC) is the new mandatory Medicaid managed care program in Pennsylvania for individuals who are eligible for both Medicaid and Medicare (dual eligibles) or who receive Medicaid-funded Long-Term Services and Supports (LTSS).  

CHC changes the way that health care, nursing home care, and services at home and in the community are paid. CHC plans will help individuals pay for and manage their hospital care, medical care, and home and community-based care. 

CHC went into effect in Southeastern Pennsylvania on January 1st, 2019. There are three CHC plans serving residents in Bucks, Chester, Delaware, Montgomery, and Philadelphia Counties – Keystone First, PA Health & Wellness, and UPMC Community Health Choices. Individuals eligible for CHC will have selected one of these plans or will have been automatically enrolled into one by the state.
