Help Yourself to Health: Chronic Disease Self-Management

Do you have diabetes, arthritis, asthma, high blood pressure, HIV, heart disease, COPD, or another chronic health condition? Help Yourself to Health gives you techniques to better manage associated symptoms like pain, fatigue, frustration, stress, and isolation. This course has been demonstrated to help older adults better manage their chronic conditions, improve their quality of life, and lower their health care costs. Participants also learn about nutrition, exercise, and strategies for communicating with friends, family, and healthcare providers.

This six-week workshop is part of the Chronic Disease Self-Management Program and is FREE to adults 60 and above. In light of COVID-19, this will be offered remotely.

Participants will receive a tool kit in the mail. Your tool kit will include a workbook, CDs for exercise and relaxation, tip sheets, self-tests, and guides for all activities in the workbook.

Participants will also join in a weekly conference call. These conference calls will be an opportunity to check in with a facilitator and other participants to discuss content in the workbook, ask questions, and monitor progress towards health goals.

The course is designed to last six weeks with the work being completed independently between the weekly check-in calls.

This program is supported by Philadelphia Corporation for Aging through Title IIID funds received from Pennsylvania Department of Aging. The Chronic Disease Self-Management Program is an evidence-based program originally developed at Stanford University and now owned by Self-Management Resource Center.
