Help Yourself to Health: Chronic Disease Self-Management

Jefferson Hospital & The LGBT Elder Initiative present “Help Yourself to Health: Chronic Disease Self-Management”

Do you have diabetes, arthritis, asthma, high blood pressure, heart disease, COPD, HIV, or another chronic illness? Help Yourself to Health gives you techniques to better manage associated symptoms like pain, fatigue, frustration and stress.

Help Yourself to Health is a 6-session course. Participants are expected to attend all six sessions:

  • March 7th
  • March 14th
  • March 21st
  • March 28th
  • April 4th
  • April 11th

 

This Chronic Disease Self-Management course has been demonstrated to help older adults better manage their chronic conditions, improve their quality of life, and lower their health care costs. Content covered as part of the course includes techniques to deal with problems such as frustration, fatigue, pain and isolation. Participants also learn about nutrition, exercise, and strategies for communicating with friends, family, and healthcare providers.
